The opening at top of chimney measures 6.5" x 14.5" and is flush with top. What choices do I have for a cap?

Because you do not have an exposed flue tile to attach to, I would recommend using a "swingtab" style of cap, which can be adhered or anchored to the surround cement wash. A good cap for your application would be the BigTop 14 in. x 21 in. Painted Black Galvanized Steel Multi-Flue Chimney Cover. This cap would give enough additional coverage to divert rain from the flue opening, while providing a barrier for pests. The cap can be mounted by using construction adhesive or by drilling the chimney wash for anchors and mounting screws.
